What is this card? 2011 ITG VIP

Bought this of eBay a few weeks ago and didn't think much of it. But the back has a persons name on the back which I believe was/is a member here? Either way, is there a checklist for this set? Looking through online lists and I can't find the card. Is it 2011-12 ITG VIP? Does the card have a number? Any help would be appreciated.

there is no checklist from what I remember you had to send in so many qualifying box tops to get a VIP member card with your name on the back ....I have Rask,Sedin and Howe

I remember those - pretty cool to get your name personalized on a card. I have a Seguin...think it is minor league team jersey.
