I don't know if anyone might be interested but here are some of the NPN addresses and requirements I have:
Starting with Donruss:

Open to only U.S./Canadian residents (except Quebec).
Handprint name and complete address on a 3x5 index card, only one index card per envelope mailed seperately.
Place in a #10 envelope (the longer business envelopes).
