Jeff it looks to me like it's from the Gretzky box set UD did upon his retirement.

That's a card from 1999-00 Gretzky Exclusives set that came as bonus packs in various 1999-00 UD Hobby products. Back in the day, me and my brother pulled a Exclusives Platinum 1/1 card from our bonus pack in a SPx Hobby box.
